It is a typical Portuguese communal flag, with the coat of arms centred on a plain yellow field (village rank).

The arms are Azure an open right hand Or surrounded in chief by two 5-point stars Argent and along the base by three globe amaranths Or. Mural crown Argent with four visible towers (town rank)(town rank) and white scroll reading "VILA DE FOLGOSINHO" in black upper case letters. (Might as well be other species of amaranth instead, or even even as a generic amaranth.)
Meaning:
I have no idea about the symbolism of the arms, apart from the stars standing for the local Star Mountains (Serra da Estrela).

Presentation of Folgosinho

Folgosinho Commune is one of the 16 communes of Gouveia Municipality (q.v.); it had 580 inhabitants in 2001 and covers 52,33 km². From 1187 to 1836 it was a separate municipality (what may account for the early attribution of a communal CoA).
