It is a fairly typical Portuguese municipal flag, with the coat of arms centred on a red and blue quarterly (town rank) background.

The arms are Argent a mound Sable issuant from the base charged with a pall wavy Azure fimbriated Argent and sprouting a fountain Azure between four towers Gules with port and windows Or. Mural crown argent with four visible towers (town rank)and white scroll reading in black upper case letters "VILA DE S.PEDRO DO SUL".
Meaning:
"São Pedro do Sul" means here ""St. Peter of South","South" ("Sul") being a nearby river. The arms show no connection with St. Peter and include symbolism about the local river fork (tributary Sul meeting Vouga river), local hot mineral spa tourism activity , and local defensive fortifications.
