Background

Isla Saona (Saona Island) is located close to the south-east tip of the Dominican Republic. On 14 September 1494, Michele da Cuneo, member of the second expedition led by Christopher Colombus, landed on the island. Called Adaymanas by its Taino inhabitants, the island was renamed after the name of da Cuneo's birth town, Savona in Italy.

Savona city flag use

According to Luis Simó in Listin Diario, 25 March 2008, the Italian city of Savona and the Dominican island of Saona have set up a partnership program. Accordingly, the flag of Savona shall be permanently hosted on the square of the village of Mano Juan, on Isla Saona, while the national flag of the Dominican Republic shall be hoisted over the city hall of Savona.

I have not found any image of the flag hoisted in Mano Juan, but we can expect to see there the official flag of Savona, that is a flag horizontally divided red-white-red.
