The municipality of Istočna Ilidža (East Ilidža) was formed in the 1990s from parts of the previous municipality of Ilidža. Initially named Srpska Ilidža, the municipality was renamed Kasindo in 2004, and eventually, in 2005, Istočna Ilidža.
The seat of administration is in Kula.

The coat of arms of Istočna Ilidža (image) is a shield coloured in graduated shades of red, blue and white bordered golden and divided by a golden cross, in the first the outlines of the Church of St. Basil of Ostrog at Veljine (in Dobrinja, Hram Sv. Vasilija Ostroškog na Veljinama), in the second the town's skyline with skyscrapers, one ensigned with a red Greek cross (representing the Hospital in Kasindo), the third divided by a serrated line representing mountain peaks (Mount Igman) and a wavy line and in the fourth a standing gravestone (stečak) ensigned with a rectangular blue field with a cross and four red shapes in white panels surrounding it. Overall, from the third field a figure of St. George on a horse slaying a dragon. On a golden ribbon under the shield is inscribed the full name of the municipality.
