I found an article online from 27 July 2015 at chillicotheohio.com that talks about the city flag and includes a photograph. According to the article, the flag was designed as part of a competition in the mid-1970s and was flown outside the city administration building until the flagpole it was on was taken down.
The flag in the photograph appears to be a standard 3:5 US-manufactured flag, divided horizontally with dark blue over red and a white triangle pointing in from the fly and shallowly swallow-tailed. In the white triangle on the fly is a black arrow pointing to the fly, superimposed by a white disc outlined in red. Inside the disc is a red capital "C" with a small five-pointed dark blue or black star.
