https://olimpijski.pl shows a vertical flag (in mourning dress) with the badge of the PKO with the name of the organization below (pl@oly20v). There is a horizontal version of the flag with text (https://zskrzywin.pl) and without text (https://pultusk.pl).

The Polish Olympic Delegation has its own flag with its logo on a white
background (https://pultusk.pl).
Created in 2019 by the branding studio "Signature", it was made not only to mark
the 100th anniversary of the PKO but also to align themselves with updated rules
on logos that could be used by organizations under the IOC umbrella (https://www.wirtualnemedia.pl).
The 100th anniversary also saw a special logo and flag made for the
occasion (https://ipapolska.pl).
There also seems to be a formal flag used for ceremonies at
https://gazetakrakowska.pl but I do not have too much detail about this
flag.

I found the flag of the Polish Olympic Committee at http://www.lubin.pl/aktualnosci,9218,natalia_dziekuje_lubinianom_foto_.html. The flag is a white cloth with the logo in the center. Below the logo is the black inscription "Polski Komitet Olimpijski" in Avant Garde fonts.
