It is a slightly untypical Portuguese communal flag, with the coat of arms centred on a plain yellow field (village rank).

The arms are Sable over a base wavy Argent and Azure, a lattice Or in chief superimposed by a Paschal Lamb Argent, in base by a bridge argent masoned Sable. Mural crown argent with four visible towers (usually meaning town rank) and white scroll reading in black upper case letters "SÃO JOÃO DO CAMPO".
Meaning:
The Paschal Lamb is an attribute of St. John the Baptist, the base wavy is probably alluding to Mondego River.
