The flag is a white field with a green border. In the center is the town seal with a red outline, with the town's name arched over it.

The seal is an artistic depiction of the Shenandoah Valley with the Appalachians in the background. The seal features some notable places of interest. The Toms Brook, which is the origin of the town's name, is seen flowing from the Appalachians, which will link up of the Shenandoah river southeast of town. A Brook is an older term used to describe a very small stream. There's also the old railroad bridge that crosses the brook as well. Next to the brook is a wooden building, which probably refers to the Virginia Military Blockhouse which the original stood from 1755 until 1770, you can read more about it here. In the foreground is a Virginia historical marker for "Action of Toms Brook," a battle from the civil war. It says "Here Early's Cavalry under Rosser and Lomax was driven back by Sheridan's cavalry under Torbert, Oct 9 1864" and is marker A-25.
