The Lubicon Lake First Nation was a Cree First Nation in Northern Alberta.
The flag is a white field with the band's emblem centred. Image above based on this photograph.

Following an internal political crisis, Lubicon Lake First Nation doesn't exist anymore. It is now split into Lubicon Lake Nation (unrecognized tribe), and Lubicon Lake Band (recognized). The flag above is used by the unrecognized Lubicon Lake Nation, and the Lubicon Lake Band is using white flag with new logo on it.
