The link above provides shows a photo of the flag. The notice can be reached via the link in the third row of the first box (page in Japanese). (URL (page in Japanese)) The original flag is 1.2m x 1.8m.
The emblem features a stylised hirgana ta た of AizuTAkada almost inside an incomplete circle, which symbolises the peace of the citizens and unlimited progress and development. I could find any notice regarding the adoption of the emblem.

Aizutakada was merged into Aizumisato Town on Oct 1st 2005and the city emblem was adopted on Dec 16th 2005 by Notice No 178.
