It is a typical Portuguese communal flag, with the coat of arms centred on a field gyronny of eight of white and blue.

Shield Argent with base wavy of Azure and Argent, issuant from base a bridge Argent masoned Sable with two arches, in chief a hurt charged with a radiant sun Or, flanked by two branches of privet Proper, the dexter one blossomed, the sinister one fruited. Mural crown Argent with five visible towers (city rank) and white scroll with inscription in black capitals "CIDADE DE ALFENA".
Meaning:
The silver colour of the shield is symbolizing hope and is referring to mining in the past of Alfena. The privet branches (Latin: Arbusto oleáceo / Port.: alfena) are canting elements, furthermore the 21 berries of the sinister one are representing the 21 settlements of the city. The bridge is simbolizing Lazarus Bridge (Ponte de S. Lázaro) crossing Leçça River, which is sinmbolized by the base wavy. The sun is simbolizing eternity, while the hurt, a blue disc is simbolizing the universe, and fidelity, joy and nobility as well.

The previous symbols had been nealy the same as current ones but the flag was quartered of blue and white with mural crown having just four visible towers (town rank) and inscription "VILA DE ALFENA".

Alfena Commune is one of the four communes of Valongo Municipality; it had 15 211 inhabitants in 2011 and covers 15.7 km². On 6 April 2011 Alfena was upgraded to a city.
