It is a typical Portuguese communal flag, with the coat of arms centred on a field quartered of blue and yellow.

Shield Or, an eradicated elm Vert between two spools Argent with yarns Azure, in chief a spear Sable with spear head Gules in fess, and in base wavy Azure two barrulets wavy Argent. Mural crown Argent with four visible towers (town rank) and white scroll reading in black upper case letters "NEGRELOS - S. TOMÉ".
Meaning:
The word "negrelo" is a variant of "negrilho", itself a regionalism for any of the more widespread synonyms for "Elm", of which there is one in the arms. São Tomé is Thomas the Apostle, the Twin, a.k.a. the doubting Thomas; one of his tokens, the spear of martyrdom, is shown in these arms. This arms are therefore canting.

Negrelos (São Tomé) Commune, a.k.a. São Tomé de Negrelos, is one of the 14 communes of Santo Tirso Municipality; it had 4032 inhabitants in 2011 and covers 6.1 km².
