It is a typical Portuguese communal flag, with the coat of arms centred on a plain yellow field.

Shield Azure, in base a railway profile Argent fixed by tirefonds Or, in chief two oak twigs Or ordered per fess with four acorns capsuled Argent, the whole surrounded by a rosary Argent set in orle. Mural crown Argent with three visible towers (village rank) and white scroll with inscription in black initials: "NOSSA SENHORA DE FÁTIMA - ENTRONCAMENTO" (left image).
WIKIPEDIA displays slightly different arms with twigs having just two acorns each and the Latin Cross of the rosary is in base and not in chief (right image).
Meaning:
The railway profile is alluding to the important railway junction placed in Encontramento. The blue colour of the shield, the rosary and, if I understood right, also the twigs are attributes of Our Lady of Fátima, the local patron saint.

Nossa Senhora de Fátima Commune is one of the two communes of Entroncamento Municipality (q.v.); it had 12 616 inhabitants in 2011 and covers 9.2 km². Nossa Senhora de Fáátima is the area north of the railway line Lisbon - Porto and was separated from São João Baptista Commune in 2004.
